Doutta Galla provides high quality, culturally appropriate and accessible primary, community and mental health services with a particular focus on the most vulnerable and disadvantaged in the cities of Melbourne and Moonee Valley.

Central Intake

Central Intake provides initial needs identification and is one of Doutta Galla's initial contact points (via e-referral, walk-in and phone queries). The initial needs identification process aims to improve the client’s experience of the
Doutta Galla service system by identifying their needs and any risk factors early in their contact and streamlining their access to all relevant services internally and externally.

The intake process includes the following

  • Establishing eligibility for service
  • Acknowledging referral by contacting referrer
  • Providing information and referral
  • Conducting client initial needs identification, including preliminary discipline assessment, general health screen, complex care screening, and discussing use of information and client rights and responsibilities
  • Prioritising the client using clinical and priority of access tool
  • Placing eligible clients on appropriate waiting list or making urgent appointments

The central intake service processes new referrals for:

  • adult occupational therapy
  • adult physiotherapy
  • paediatric occupational therapy
  • paediatric physiotherapy
  • paediatric speech pathology
  • dietetics
  • counselling
  • diabetes education
  • podiatry and
  • mental health intake (standard and moderate outreach, social inclusion programs and residential services - Rocket, Chiron and Janoak).

How to refer a client to Doutta Galla

A referral can be made by any individual (including a self-referral) and by any service.

Electronically

Doutta Galla's preferred method of referral is electronically, via the Connecting Care website

Our central intake team will receive referrals sent via Connecting Care. The exact referral method for each service is shown within the Connecting Care website. The Connecting Care website requires the creation of a user name and to log in. The website also offers a user guide for first time users.

Fax Referrals
Referrals can be sent via fax (9374 2866). Service Coordination Tool Templates/profiles (SCTT) are used to make a referral. All faxed referrals should include:

Confidential referral coversheet
Consent to share information
Consumer information
Summary and referral information

Note: With client consent, please include other relevant information, such as general or specialist medical correspondence, care plans, and risk plans.
