Doutta Galla Chronic Disease Resource Centre

The Doutta Galla Chronic Disease Resource Centre provides the community and health providers with information, resources and tools to help in the management of the following long term health conditions:

1. Arthritis
2. Diabetes Type 2
3. Heart disease
4. Lung conditions such as Asthma and Chronic Obstructive Pulmonary disease

What is a chronic disease?

Chronic disease otherwise known as a chronic condition is a long term health condition that people live with for more than six months.
Doutta Galla Community Health provides people living with one or more of the long term health conditions (mentioned above) with ways to better manage their health.
Given the challenges faced when living with a long term health condition(s), care coordinators are available to assist you in the development of skills to better manage your health and get the most out of your life.
Eight out of ten of our clients are living with a chronic condition. A chronic condition is a prolonged condition which requires ongoing treatment, or treatment for a period of at least three months. Examples are asthma, diabetes, heart disease and arthritis.

Early Intervention Chronic Disease Management (EICD)

Doutta Galla Community Health received funding in 2009 to increase service delivery to people with chronic disease and build in new components of care that are consistent with evidence based chronic care. DGCH focusses upon community based early intervention services for people with chronic diseases to make it easier for people living with chronic conditions to manage their general health.
