Assessment

Accurate diagnosis and assessment is important to initiating appropriate treatment and optimising ongoing care. Common assessment forms should collate minimum data for all chronic conditions, including identifying lifestyle risk factors, self-management skills and depression.The Chronic Care Model also highlights the importance of exploring client's perceptions of their health problem, fears and priorities for care as fundamental to the assessment process.

What is a comprehensive assessment?
Assessment is a decision making methodology that collects, weighs and interprets relevent information about the client. There are three types of assessment: service specific, specialist and comprehensive.

Who gets a comprehensive assessment?
Clients with one or more of the following long term health conditions:

  • Arthritis
  • Diabetes Type 2
  • Heart disease
  • Lung conditions such as asthma and chronic obstructive pulmonary disease (COPD)
  • Plus the client is interested in:
  • Support with behaviour/lifestyle change, and/or
  • Self management support, and/or
  • Assistance with care coordination because multiple services are involved.

Chronic Conditions Assessment and Support Tool (C-CAST)
The Chronic Conditions Assessment and Support Tool (C-CAST) is a health screening tool. C-CAST supports a client's self-management of their chronic condition(s). This is the focus of the Early Intervention in Chronic Disease (EIiCD) at Doutta Galla Community Health (DGCH). It is used by service providers from clinical, aged and chronic care, mental health, and outreach and complex needs in order to:

1. Assess their clients’ self-management ability.
2. Screen for depression and anxiety.
3. Meet minimum standards for chronic disease management.

  • keep track of appointments and;
  • share important health information about your care
