The Central City Community Health Service is operated by Doutta Galla Community Health in partnership with a range of co-located health and support agencies providing services for people living in or accessing the City Of Melbourne who are experiencing homelessness, people who are at risk of homelessness, or living in low cost accommodation. This service provides an integrated model of care servicing clients with complex health and social needs by providing outreach and site based programs and services.
We are seeking an experienced and dynamic allied health professional to fill a brand new role. You will be a committed and highly organized professional who demonstrates a sound understanding of the key issues of and experience in providing services to clients who have a range of complex needs.
The role will be responsible for the day-to-day clinical care and assisting development of a growing outreach allied health team supporting service innovation, team building, partnership development and ensuring the delivery of high quality clinical services as part of an integrated service delivery model.
