Diabetes Education

Diabetes chinese community

The Diabetes Assessment and Education Service is provided by cohealth’s Diabetes Education team at the Niddrie and Kensington sites.

This team includes:

  • Diabetes Educator: Introduction to diabetes self management; help and support to manage your diabetes including education, self blood glucose checking, monitoring your diet, medications and the routine health screening.
  • A General Practitioner: A doctor who will monitor and manage your health and medical needs; and involve you in managing your diabetes (Kensington only).
  • Dietitian: Nutrition assessment; review of eating habits; advice on dietary recommendation for diabetes.
  • Podiatrist: Assessment of nerves and circulation to your feet; foot health check; advice on foot care and footwear.
  • Physiotherapist: to assist with your physical activity and maintaining a healthy strong body.
  • Counsellor: To assist with stress management and adjusting to living with diabetes

The service is for clients with diabetes, their families and carers. It is open to clients with newly diagnosed diabetes or those who have had diabetes for a long time.

Although you do not need a doctor’s referral to attend this service, we recommend you discuss your diabetes management with your doctor. It assists us at the clinic if you bring your medications as well as recent blood test results. Your doctor will be happy to provide you with a print out of this information.

Group Programs

Diabetes management
It includes an overview on diabetes management by a Diabetes educator, Dietetian, Physiotherapist and Podiatrist.

Diabetes, Stroke and Heart Disease Prevention "Life!" Program

Life! Logo

This program is designed for people who are at risk of having type 2 diabetes and cardiovascular diseases. Discuss with your doctor about the Type 2 Diabetes Risk Test and if you are eligible for this program.

This program consists of five sessions, 90 – 120 minutes each, over six months. The goal is to work toward a weight loss of at least five per cent of body weight through a better diet and 30 minutes of moderate exercise every day.

Home visits

This is a restricted service for clients who are permanently house bound and unable to leave their home. Phone the Diabetes Educators (Kensington: 8378 1600; Niddrie: 83783500) if you require a home visit.
