Outreach provides:
- Individualised support, tailored to particular needs and goals
- A service which works with participants in their home and local community.
- Support and assistance across a wide range of areas including; personal development, daily living, accommodation, leisure and recreation, social, education and training and includes the following programs:
Multiple and Complex Needs program (MACNI)

Intensive Home-based Outreach programs
The Intensive Home Based Outreach Program offers a pathway out of homelessness for people who have a mental illness and complex needs.
The program is for people who
- Experience a psychiatric disability or who have a history of involvement with mental
- health services
- Are homeless and eligible for public housing
- Who have complex needs
- Who are over 18 years old
- Have needs currently not being met by other services
- Who are open to entering into an ongoing support plan Read more.......
Home Based Outreach and Housing Support Program
Outreach with housing provides: Housing is provided which may be shared and single, with individualised support, tailored to particular needs and goals.This housing is permanent.
