What Does The Community Health Nurse (CHN) Do? In partnership with you and your Doctor, the Community Health Nurse complements and works to create a healthy and happy environment for you as an individual, your family and any groups that you may belong to.
What kind of services does the CHN provide ? A variety of Programs & Services in both the local areas and/or within DGCHS are available to you, either directly or through a referral by the CHN.
How is this done ? A general assessment done by the CHN is followed by a discussion with you regarding your individual needs. With your consent a referral is made to the most appropriate Service Provider. |

MORE SPECIFICALLY SOME OF THE PROGRAMS / SERVICES OFFERED BY THE CHN ARE:
- Diabetes Prevention/Management
- Cardiac Rehabilitation
- Aged Care Support & Recreational Services
- Community Asthma Program (for 0 to 18 year olds).
- Home Based Falls Risk Assessments
- Nurses Clinic. Compliments the Medical Service)
- Stroke Prevention Program, for people who have recently been discharged from the Royal Melbourne.
Do you have to wait for this service ? No, but you may have to wait for the service that you maybe referred to depending on the initial assessments.
Are you eligable to the CHN services ? Community Health Program, this service is available to anyone who works, lives and / or studies in the Cities of Moonee Valley &Melbourne.
How can you get to see a CHN ? Referrals can be made by a phone call, fax or e-referral to the DGCHS Intake Worker by:
- Self.
- Local Doctors.
- Community Based Health Service
- Professionals.
Home Visits If you cannot get to the Health Centre to see the CHN, home visits can be arranged at a time that is convenient for you and/or your family. If a referral to other services is required this will be discussed and organised for you after the initial assessment.
