Doutta Galla Community Health Service’s Diabetes Education team at the Kensington Site provides the Diabetes Assessment and Education Service.
As a team with Dietitian and Podiatrist, Assessment, education and support to people who is newly diagnosed/ unstable with type 2 Diabetes Mellitus; general support to people with type 1 DM. Includes both individual and group sessions along with some home visit as well as education sessions for the community. Yearly review clients with diabetes

Occupational Therapy
In the community setting the Occupational Therapist (OT) is mainly involved in performing client home visits to provide education and make recommendations on increasing home safety and independence for clients and carers. This involves education and demonstration of alternative techniques and/or assessment and prescription of equipment.
The community OT role is primarily a consultancy, client advocacy type role. The OT is usually involved short term to assess clients, provide recommendations and complete follow-up (including referrals to other services, completing applications for equipment to funding bodies and completing letters of recommendation)

Podiatry Services funded by DHS Community Health Program are accessible to clients who live, work or study in the cities of Moonee Valley and Melbourne.
Podiatry Services are accessible across all DGCHS sites.
