DIETITIAN

The Dietitian provides information on foods suitable to help improve your medical condition, and increase your individual health and well being.

You may be seen individually or in groups and the service includes education / information on issues related to:

  • Overweight
  • Underweight
  • Obesity
  • Diabetes
  • High Cholesterol
  • Healthy Eating
  • Heart Disease
  • General nutrition counselling (HACC and CH)
  • Diabetes Services:  one on one counselling and groups (inc diabetes shopping tours)
  • Café Meals Program
  • Outreach service (including mental health and homeless)

HOW TO ACCESS A DIETITIAN?


Dietetic services are accessible at all DGCHS sites and appointments can be made at any site.

HOW LONG DO YOU HAVE TO WAIT BEFORE YOU CAN SEE A DIETITIAN?

Varies with site requested.Usually clients seen within 1 month of referral.


WHAT WILL DIETITIAN’S CONSULTATION INVOLVE?


The Dietitian will initially identify your individual / family dietary intake and inform you of foods that can be modified and / or replaced to contribute to optimum health.

A Supermarket Tour is periodically organised to:

  • Identify the types of foods available today which contribute to the confusion in modern day diet
  • How to read labels
  • Buy the right foods that contribute to good health.

WHO IS ELIGIBLE FOR THE DIETETICS SERVICES?
This service is available to anyone living, working and/or studying in the Cities of Moonee Valley and Melbourne.

The Dietitian will see anyone who:

  • Refers themselves or by their family and friends
  • Is referred by their local doctor or other local community based organisations.
  • Is referred by other DGCHS staff.
  • Is referred by the local hospitals.

Initial appointment may last up to one hour and ongoing appointments up to 30 minutes.

CAN YOU GET THIS SERVICE IF YOU ARE HOUSE BOUND?
If you are unwell and cannot get to the Centre for your appointment, a Home Visit can be organised. This is a restricted service for clients who are permanently house bound and unable to leave their home. Access to a home visiting service is determined  by the Dietitian.
