Diabetes education
Doutta Galla Community Health Service’s Diabetes Education team at the Kensington Site provides the Diabetes Assessment and Education Service.

As a team with Dietitian and Podiatrist, Assessment, education and support to people who is newly diagnosed/ unstable with type 2 Diabetes Mellitus; general support to people with type 1 DM. Includes both individual and group sessions along with some home visit as well as education sessions for the community. Yearly review clients with diabetes.

Appointment Required:  Yes

Eligibility: 
Clients must live, work or study within the City of Moonee Valley or Melbourne catchment areas.

Indicative waiting time:
Variable from one to three weeks.

 

Exclusions:

Gestational Diabetes; Newly diagnosed Type 1 Diabetes Mellitus; Children with type 1 DM; Insulin Infusion pump.
