Housing Support for the Aged

The Housing Support for the Aged Program (HSAP) aims to improve the health and well-being of older people with unmet support needs who are applicants for public housing. This is achieved by assisting clients to maintain their housing and independence. The program monitors clients well being, provides and/or purchases care and support and ensures clients obtain access to additional health, welfare and community services. HSAP uses flexible service response funds to purchase good and services when specialist and mainstream service systems are unable to adequately meet client need. Read more ......
 

HARP Psychosocial

People who have complex needs and or significant complications as a result of chronic disease including, but not limited to; heart disease, chronic respiratory disease and diabetes who:
-have presented to or been admitted to the Royal Melbourne Hospital (or other nearby hospital (not mandatory for diabetes services)
-are at imminent risk of hospitalisation
-live in the catchment areas

 Older Persons High Rise Program 

To improve the health and well-being of tenants aged 55 + living in direct tenure housing at 29 Crown St, Newmarket and the Kensington Estate.

This program assists tenants living in high-rise housing. A Community Support Worker is located at each site. Activity programs are designed for and advertised at each high rise.

Objectives
1. To ensure isolated and vulnerable tenants have access to support and services.
2. To contribute to tenants’ sense of safety, independence and security.

Dietitian-Outreach/Cafe Meals program

The Complex Needs Program dietician is responsible for quality outreach and community based dietetics services to people who are homeless, at risk of homelessness and have complex needs within the Cities of Moonee Valley and Melbourne.

Café Meals Program
The Café Meals Program is a subsidised meals program administered by the Complex Needs Program (CNP) at Doutta Galla Community Health Service (DGCHS). The program aims to provide increased access to safe, nutritious, affordable meals for people who are isolated, homeless or at risk of homelessness. The program does this by providing a social dining experience for otherwise socially isolated people, at cafes that provide nutritious meals at affordable prices. Read more.........

 
Outreach Podiatrist

Complex Needs Program podiatrist is responsible for quality outreach and community based podiatry services to people who are homeless, at risk of homelessness and have complex needs within the Cities of Moonee Valley and Melbourne. This includes assessment, diagnosis and treatment of clients with foot related /lower limb disorders.  Read more........
