Making a Referral 

Doutta Galla Community Health Service accepts referrals electronically from service providers via the Connectingcare website.   Referrals sent via Connectingcare will be received by our Intake Team.  Some services accept discrete referrals directly.  The exact referral method for each service is shown within the Connectingcare website.
