Becoming A Member

Membership is open to any adult who uses the Community Health Service or any adult who lives, works, or studies in the Cities of Moonee Valley or Melbourne, or if the person is a volunteer of the service.  Membership is free.

If you would like to become a member of the Doutta Galla Community Health Service please fill out an "Application for Membership Form".  Once you have completed the information you may submit the form to the Executive Assistant to the CEO (The details are listed at the bottom of the form).
