Older Persons High Rise Program (OPHRP)

External Referral
External referrals can be made directly to the OPHRP Worker. The OPHRP worker contacts the client, completes the assessment and where appropriate and registers the client

Cost
There are no fees charged by the Program.
A contribution may be requested for some activities arranged by your Community Support Worker.

OPHR staff operate Monday - Friday, 9am – 4.30pm from:

Eligibility: 
Frail or disabled older people, younger people with disabilities and their carers who are residents of the high rise estates:

Need to be eligible for HACC services.

Intake point and process
There are four ways clients are referred to the program:
1. Assertive outreach/active engagement by Community Connections Program worker
2. Self referral
3. Internal referral
4. External referral
