Outreach Podiatry

Complex Needs Program podiatrist is responsible for quality outreach and community based podiatry services to people who are homeless, at risk of homelessness and have complex needs within the Cities of Moonee Valley and Melbourne. This includes assessment, diagnosis and treatment of clients with foot related /lower limb disorders.

Eligibility: 
People who are homeless, at risk of homelessness or living in low cost accommodation who are experiencing issues with their feet and/or lower limbs
•HACC eligible
•Homeless, at risk of homelessness or living in low cost accommodation

Criteria for prioritisation
People experiencing homelessness

Intake point and process
There are four ways clients are referred to the program:
1.Assertive outreach/active engagement by Podiatrist
2.Self referral
3.Internal referral
4.External referral

Service availability
Service operates on an assertive outreach model and is based at:
15 – 19 Gracie Street, Nth Melbourne
Thursdays and Fridays, 9am – 4.30pm.
