| Who can access this service?
The Intensive Home Based Outreach Program offers a pathway out of homelessness for people who have a mental illness and complex needs.
The program is for people who
- Experience a psychiatric disability or who have a history of involvement with mental
- health services
- Are homeless and eligible for public housing
- Who have complex needs
- Who are over 18 years old
- Have needs currently not being met by other services
- Who are open to entering into an ongoing support plan.
This program offers:
- Flexible, intensive and individually tailored assertive outreach support
- Access to transitional housing during the program
- Assistant for people to secure long-term housing and to improve their health and wellbeing
The program worker will develop a plan with the individual to address the individual goals. Some of the issues the program can assist with include: legal, financial, accommodation, health, social isolation and recreation
This program is funded by the Department of Human Services. There are no fees charged by the Program. A contribution may be requested for some activities/items arranged by your Community support Worker.
