Outreach with housing provides: Housing is provided which may be shared and single, with individualised support, tailored to particular needs and goals. This housing is permanent.
Eligibility: -Outreach with housing is available to adults who: Have a major psychiatric disability which affects their ability to live independently in the community -Want support to develop goals and to assist with living independently in the community Outreach with housing is available to adults, 16 years and over who: -Are eligible and registered for Office of Housing. -In shared housing the applicant must be willing to cooperate with co-tenant/s and support workers in regards to all matters of household management.
-Reside in the City of Moonee Valley or Melbourne
People making application to this program are expected to accept outreach support for a minimum of two months prior to any offer of housing.
Support and assistance across a wide range of areas including;personal development, daily living, accommodation, leisure and recreation, social, education and training.
Intake point Sent to Coordinator of Outreach, then assessment done by team leader, and – when housing is available- allocated to a support worker.
